[Hiring] Innovation Program Director @DaVita Kidney Care

Role Description

This position is responsible for executing on tactical, analytical and strategic work necessary to ensure that the model of care team meets clinical and performance goals and has the greatest clinical impact. The position will significantly contribute to shaping the future of integrated care at DaVita.

• Provide strategic direction and tactical oversight for model of care initiatives to drive program goals (i.e. broaden reach, improve clinical impact).
• Coordinate program team and program initiative plans.
• Develop strong relationships with internal and external partners.
• Monitor and manage model of care program performance.
