Role Description

Wider Circle is hiring an Associate Care Advocate to join our Care Delivery team. This is an entry-level job with a clear path to move into a more senior role.

In this job, you will:

• Call patients by phone to learn what challenges affect their health.
• Help connect them with services and keep clear, accurate notes about each call.

We are looking for someone who is kind, organized, and excited to help people in their community. You will do well in this role if you:

• Care about helping others and want to reduce health inequity.
• Build trust quickly, listen well, and communicate clearly.
• Stay organized under pressure and take privacy and accuracy seriously.

Qualifications

• A high school diploma or GED is required.
• A degree in Human Services, Public Health, or a related field is a plus.
• Speaking both English and Spanish (or another language) is strongly preferred.
• A Community Health Worker (CHW) certificate is preferred.
• Experience in healthcare, social services, or community work is helpful but not required.
• Comfortable speaking with many different types of people by phone.
• Comfortable using websites and apps to enter and track data.
• Able to adapt in a fast-moving work environment.
• Able to manage your own tasks, stay on top of notes, and meet goals without close supervision.
• Willing to complete our onboarding program, which includes call practice, documentation exercises, and role-play.
• Willing to complete a drug test and background check before starting.

Requirements

You must be able to:

• Sit, stand, and walk as needed.
• See and hear well enough to speak with patients by phone and in person.
• Type and use multiple computer programs at once while talking.
• Have a strong internet connection and a quiet, private space for patient calls.
