[Hiring] Aftercare Recovery Worker @Change Grow Live

Role Description

We are seeking a dedicated Aftercare Recovery Worker to join our Bi Borough Team.

Responsibilities include:

• Support the service user to connect with activities, sustain recovery, and reach their goals set out in their aftercare plan (e.g., training, employment support).
• Facilitate post-treatment groups across the Bi Borough in community settings, outside of CGL hubs, to support individuals in progressing their recovery journey independently of services.
• Provide service users with information on options available to them within the service and in their communities.
• Collaborate with the Partnership Lead to engage partner agencies that service users can access for support following treatment.
• Build positive relationships with aftercare providers to ensure recovery support is available to support post-Rehab early abstinence (e.g., Build on Belief (BOB), Fellowship Groups, Aspire2be, SMART).
• Work collaboratively with the Service User Inclusion Worker to understand post-treatment needs, ensuring the voices and preferences of service users are heard and reflected.
• Work with Recovery Coordinators, Peer Mentors/Volunteers to provide Recovery Check-Ins.

Qualifications

• Excellent knowledge of inpatient detox and residential services.
• Experience of working within multidisciplinary teams and fostering positive relationships with partner agencies and external providers.
• Experience of working with people at all stages of their treatment journey and an in-depth knowledge of the importance that group work and aftercare support offers.
• Excellent written and verbal communication skills and the ability to provide effective advocacy when needed.

Requirements

• Full-time hours: 37.5 per week.
• Contract Type: Fixed Term up to end of June 2028.
• This role is not eligible for visa sponsorship; applicants must already have the right to work in the UK at the time of application.
